Well Driller Certification Program

This site will no longer be accessible for Well Completion Reports after July 2010. Please use the eDEP Well Completion Forms which are available on-line. On-line well completion forms can only be submitted after obtaining a User ID, Password and Completing and submitting a Well Driller Proof of Identity Form. In addition Any printed copies will no longer be accepted at the DCR address. Printed forms may be submitted to MassDEP, Well Driller Program, 1 Winter St., Boston, MA 02108 for the next month. After that time, use printed forms available for download from the Well Driller Home Page or request from the MassDEP Well Driller Program. Printed forms will take additional time to process and will not have a well completion number until processed. 1. To submit or view electronic well data a MA registered driller must have completed the Updated Well Reporting Requirements training program offered by DEP and the Massachusetts Office of the State Geologist.
2. Signed DEP's Acknowledgement and Agreement to E-Filing Policy and Disclaimer to receive his/her personal identification number (PIN) to enter into system.
3. Have access to a computer, printer, modem, Internet service, web browser, and an active email account. It is recommended that you have the latest version of
Adobe Reader (which is free and available on the internet) to view the report.

Before starting, please note:

• When creating a new well report on this system, a driller will see multiple screens, each of which has required fields for the well type and work performed. These fields must be correctly filled out before the driller can proceed to the next page.


• You must be prepared to complete the report entirely within one session. Unfortunately at this time the system cannot store incomplete reports and make them available to you for editing later. The session will time out after 30 minutes if left unattended.


• When data entry is complete and the driller submits the report to DEP, he/she will receive a confirmation number, verifying that DEP has received the report and which must be used to retrieve the report electronically in the future. Please make a note of this number.


• Once data entry is complete and your report is displayed, you must print a copy for the Board of Health in the municipality in which you worked, and deliver it to them as required by CMR 313 3.03(3).


• DEP will periodically examine the submitted records. If no errors are found, the records will be flagged as complete, complete records will be searchable. Any errors will be reported to the driller via email or phone. In order to edit the report, the well driller must contact the Well Driller Certification Program at Stephen.Hallem@state.ma.us
